We care for patients who are homebound due to physical conditions. We also attend to treatments such as wound care, therapy and other services.

SKILLED NURSING

Patient assessment, education, and skilled medical care.

PHYSICAL THERAPY

Assist in the functional rehabilitation of the patient.

OCCUPATIONAL THERAPY

Training on activities and independent activities of daily living.

HOME HEALTH AIDE

Assist with Daily activities per the Nursing and Physician directions.

MEDICAL SOCIAL WORKER

Evaluate and assist with the patient’s environment, community resources, and financial needs.

SPEECH THERAPY

Assessment on difficulty swallowing, communication and speech training.

INFUSION THERAPY

Deliver fluids or medication directly into the vein.

TOTAL PARENTAL NUTRITION

Nutrition or fluids directly via an IV line.

In coordination with your physicians, we will do a complete assessment of your needs and provide necessary care to achieve the optimum outcomes for our clients. We will teach you and your family skills you need to help you recover faster. We can also assist you with medical equipment, supplies and referrals to other community agencies when needed.
